The nominator must provide the information on the online nomination form below.

Only one online form is required per nomination. A supporter of a nomination does not have to fill out the online nomination form.  

The following documents are to be uploaded to the form as part of the nomination package:

1. Nomination letter (up to 500 words)
2. Curriculum Vitae (CV)
3. 2-5 reference letters
